Copay plans are designed for individuals and families who prefer the convenience of
copay benefits for routine health care expenses. The copay amounts range from
$10-$40 depending on your selected plan.
Depending on the plan that you select, you will be able to pay a single one-price copay
fee when you visit the doctor for routine and well care visits. The plan will then pay
